We are pleased to announce Maryland Primary Care Physicians’ participation in the University of Maryland Quality Care Network (UMQCN). The UMQCN is a physician-led network comprised of employed and independent community primary care providers. The goals of the UMQCN are to improve health care quality, reduce health care costs, and to increase both patient and provider satisfaction.
The providers in the UMQCN have access to an array of additional resources for our patients – nurse case managers, social workers, clinical pharmacists and health coaches. These members of our extended care team will work directly with patients and coordinate with the patient’s primary care provider to develop individualized care plans. These care team members will engage with patients beyond the primary care physician’s office to provide additional, needed services.
The UMQCN will partner with insurance companies – commercial, government and employers – to move the providers in the network from a fee-for-service model to value-based care. The UMQCN is seeking collaborative relationships to move toward greater accountability for the quality and total cost of care for patients within the network.
