We are pleased to announce Maryland Primary Care Physicians’ participation in the University of Maryland Quality Care Network (UMQCN). The UMQCN is a physician-led network comprised of employed and independent community primary care providers. The goals of the UMQCN are to improve health care quality, reduce health care costs, and to increase both patient and provider satisfaction.
The providers in the UMQCN have access to an array of additional resources for our patients – nurse case managers, social workers, clinical pharmacists and health coaches. These members of our extended care team will work directly with patients and coordinate with the patient’s primary care provider to develop individualized care plans. These care team members will engage with patients beyond the primary care physician’s office to provide additional, needed services.
The UMQCN will partner with insurance companies – commercial, government and employers – to move the providers in the network from a fee-for-service model to value-based care. The UMQCN is seeking collaborative relationships to move toward greater accountability for the quality and total cost of care for patients within the network.
On December 15, 2015 our MPCP Patient Portal was upgraded to version 2.0. Click here to see the new more user-friendly look and functionality of our updated portal page. Our Patient Portal is a secure online website that allows our patients 24/7 access to their personal health information by using a secure login and password. Through the Patient Portal, you can schedule appointments and request prescription refills, and receive information to your portal account, including clinical summaries of your visit, patient education, lab letters and referral requests. Once you receive our email invitation, it’s easy to enroll as a Patient Portal user on our MPCP website. Now that the Portal is available for all of our practice locations, all patients for whom we have an active email are being invited to enroll, so make sure your correct email address is on file with your MPCP office. MPCP employees, friends and family are again participating in this year’s 7th Annual ZERO Prostate Cancer Run on Sunday, September 29th at Towson University’s Johnny Unitas Stadium.
