For patients with certain medical needs, MPCP offers healthcare services in the comfort and privacy of their homes. Whether they are recovering from surgery, require physical therapy or need long-term care to manage chronic conditions, our managed care team can help patients live long, full lives as independently and safely as possible.

Services offered by MPCP
Home health
Home health provides in-home care typically following a hospital stay, surgery or change in functional abilities due to illness or injury. Home health provides any or all of the following: nursing, physical therapy, occupational therapy, home health aide, social work, speech therapy, and other programs and services that are specific to the patient’s condition. Lab work can be drawn in the home. Visits are anywhere from 1-7 times per week at about an hour per visit per clinician visiting the home. Home health clinicians work under the orders of a supervising provider and can provide wound care, IV medications, oxygen therapy, medication management, and education regarding the disease process and stabilization.
Who covers home health?
Medicare and Medicaid cover home health at 100%. All other commercial insurances cover a portion of home health with remaining balances due in the form of copay or deductible.
Palliative care
Palliative care is an interdisciplinary medical caregiving approach aimed at optimizing quality of life and easing suffering among people with serious, complex illnesses. (It used to be considered “pre-hospice”, but this has changed in recent years.) Palliative care is geared at reducing symptoms from multiple, complex diseases while still allowing the patient to opt for curative measures. Services are provided by a nurse practitioner, registered nurses, physical and occupational therapy, social work, chaplain, family support/counseling, pain management, and alternative therapies such as music therapy, massage therapy and Reiki.
Who covers palliative care?
Medicare and Medicaid cover at 100%. Other insurances cover varying portions with some cost left over as copay or deductible, though most palliative care programs waive remaining fees.
Hospice care
Hospice care is a type of health care that focuses on easing a terminally ill patient’s pain and symptoms and attending to their emotional and spiritual needs at the end of life. Hospice care prioritizes comfort and quality of life by reducing pain and suffering. Services provided include a doctor to oversee care/place orders, nursing, music therapy, art therapy, massage therapy, social work, grief counseling for the patient and family (before and after death), chaplaincy, and more depending on the specific company’s offerings.
Who covers hospice?
With the exception of inpatient hospice at a center, hospice is covered by all payers at 100%.

Care management is a team-based, patient-centered approach designed to assist patients and their support systems in managing medical conditions more effectively. Care management at MPCP is comprised of registered nurses and clinical support staff ready to help our patients live long, full lives as independently as safely possible. We help with a wide array of medical diagnoses, psychosocial concerns, and socioeconomic challenges, and help conquer these barriers to prepare the patient and their support systems for success.
