Position Summary:
We are seeking a patient-centered, team-oriented Ambulatory Care Pharmacist to join our multidisciplinary primary care team. This role is critical in supporting our population health initiatives, focusing on medication management, chronic disease optimization, and health outcomes improvement for diverse patient populations. The pharmacist will work collaboratively with primary care providers, care managers, and other healthcare professionals to identify and address medication-related needs, promote evidence-based prescribing, and close care gaps across our population.
Key Responsibilities:
- Clinical & Direct Patient Care:
- Provide comprehensive medication management (CMM) for patients with chronic conditions such as diabetes, hypertension, hyperlipidemia, COPD, and heart failure.
- Conduct medication therapy reviews and develop personalized care plans in collaboration with primary care providers.
- Identify and resolve drug therapy problems, improve adherence, and optimize therapeutic outcomes.
- Provide patient and caregiver education on medication use, side effects, and lifestyle modifications.
- Population Health Support:
- Utilize population health tools and electronic health records to identify patients with high-risk medication needs, gaps in care, or suboptimal therapy.
- Participate in care team huddles and interdisciplinary case reviews.
- Support initiatives such as transitions of care, comprehensive medication reconciliation, and reducing hospital readmissions.
- Assist with Medicare patient education efforts surrounding selection of Medicare Part D coverage.
- Contribute to quality improvement efforts targeting HEDIS, CMS Star Ratings, and other performance metrics.
- Program Development & Team Collaboration:
- Partner with clinicians to develop protocols and guidelines for chronic disease management and preventative care.
- Assist with implementing and evaluating pharmacy-led population health programs.
- Train and mentor students, residents, or pharmacy technicians as appropriate.
- Administrative Duties:
- Document clinical interventions and outcomes in the electronic health record.
- Track and report metrics related to pharmacist interventions and population health initiatives.
- Stay current on clinical guidelines, payer requirements, and regulations impacting ambulatory pharmacy services.
Qualifications:
- Education & Licensure:
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
- Current pharmacist license in Maryland, board certification (BCACP, BCPS, or similar) preferred.
- Board certification within one year of hire (if new graduate)
- BLS/CPR
- Experience:
- Minimum of 2 years in ambulatory care or clinical pharmacy practice; completion of a PGY1 residency (PGY2 in Ambulatory Care preferred) is highly desirable.
- Experience working in value-based care models or population health environments is a plus.
- Skills:
- Strong clinical knowledge in chronic disease pharmacotherapy and pediatric acute and chronic disease pharmacotherapy.
- Effective communication and collaboration skills within interprofessional teams.
- Proficient in electronic health records and population health tools (e.g., Epic, Care Management Platforms).
- Data-driven with the ability to identify trends, measure outcomes, and drive quality improvement.
Work Environment:
- Outpatient clinic setting.
- May require occasional travel to affiliated clinic sites or community events.
- Hybrid work options may be available depending on program needs.
